Instructor Patient Chart Guide
Chart Basics in Student View
- To access Student View, on your home page, locate the activity under its corresponding course and click the green "Student View" button to the right of the activity. Alternatively, go to the Courses menu on the left sidebar for a better layout of your courses, click "Manage Course" for the relevant course, then find the activity and click the green "Student View" button to its right.
- After selecting Student View, choose a patient to view and interact with their chart by clicking "Patient Chart" or "Create Patient" (for new patient activities).
- After clicking on "Patient Chart", you will be taken to the Visit History page. This is the first page students see when accessing a patient's chart.
- The Patient Header contains all relevant Patient Details, including Name, Age, Date of Birth, Medical Record Number (MRN), Sex, and Allergies. The "Return to Activity Dashboard" button at the top right will take you back to the Activity.

The light blue area of the Patient Header displays Current Visit Information and Visit Navigation options. If there's no active visit, you'll see a " New Visit" button on the left side. For an ongoing visit, an " End Visit" button appears on the right side.
- The Left Sidebar contains all of the Chart's Sections, including flowsheets, forms, history, and more. You'll see two types of sections:
๐ฅ Patient History Flowsheets:
Always visible and compile information from all data, whether in a Visit or not.
Examples include:
- Patient Details
- Visit History
- Allergies
- Patient History
๐ Visit Based Flowsheets:Only visible depending on the Visit Type selected. The documentation created is tied to the Visit and will only appear on flowsheets or sections within that specific Visit.
Examples include:
- Notes (e.g., Admission Note, Progress Note, Discharge Summary)
- Orders
- MAR (Medication Administration Record)
- Vital Signs
- Intake & Output
- Assessment (e.g., Head-to-Toe Assessment)

If you're accessing a pre-built template patient in Student View for a Simulation activity or a Template Patients activity with a fixed start time, you may see a Scenario Clock. This will appear as a black clock at the top of the screen. All dates & timestamps within the patient will reference this Clock if it is available.
Visit Navigation in Student View

To create a New Visit, click the " New Visit" button in the Patient Header. Note that this option is only available if there's no current active visit or if you've ended the previous visit.

Select the Visit Type, enter the Reason for Visit, confirm or adjust the Visit Start Date, and click Save.

The light blue Patient Header will change to reflect the Current Visit Information and Visit Navigation buttons.You'll see the following Visit Navigation buttons on the light blue header:๐ Manage Visit:Edit the current visit's Start Time and other details.๐ End Visit:End the current visit and enter the Visit's End Date.

Once you've created a Visit, you can click into any section on the left and begin documenting.

Navigate to the Visit History page to review all historical visits.

Use the View Visit button to easily switch between Visits. When you click the View Visit button, the left sidebar will update to show flowsheets and sections for that specific Visit Type. A notification will appear in the top right corner, confirming you've switched to a different visit.
- When reviewing a historical Visit, you'll see the Close Visit button:
๐ Close Visit:Switch back to the Current Visit.
Documentation Specifics in Student View
- To document in ChartFlow while in Student View, navigate to a flowsheet and click "New Entry" to begin.
- Set the timestamp for your entry. Note that entries must have unique timestamps, with the exception of orders. For instance, you can't record two sets of Vital Signs at the same time. If you attempt to submit a duplicate timestamp, the system will reject the second entry.
- You'll encounter the following field types when documenting:
๐ Free Text:Both single line and multi-entry text fields.๐ฝ Dropdowns:Single Select (only one entry can be selected) and Multi Select (select all that apply).โ๏ธ Checkboxes:Check all that apply.
- Enter the required information in the appropriate fields. When navigating through a flowsheet, we recommend using Tab to quickly move to the next field.
- After entering all necessary information, click "Save" (always found at the bottom right) to complete your entry.
- For certain fields, you'll see an Actions button after saving:
โก Actions:A green button that appears after initial documentation in fields like Allergies, Immunizations, Orders, MAR, and Appointment scheduling. It allows you to add further details or update the status of your entry.

- Open the correct patient based on their MRN
- Scan the patient's wristband
- Scan the medication
- Fill out the administration details
